License Verifications. Licensee information is available at the Board's website at Alphabetical Listing of Oregon Optometric Physicians.The Oregon Board of Optometry will provide written verification of a licensee's license status with the Board upon written request. There is a fee of $20 per license verified in writing. Payment must be received before the verification is processed.

Licensee Lists. Computer printout lists, disks, or electronic files of Oregon licensed optometrists are available upon written request. There is a fee of $25 per copy of the file of either Active Status or Inactive Status Oregon licensed optometrists. You may use this order form, but any written request will be honored. Payment must be received before the list is processed.
