ALPHABETICAL LIST OF CURRENTLY LICENSED OPTOMETRIC PHYSICIANS IN PDF FORMAT
    "ACTIVE" Status = Licensee is current with regard to appropriate fees, continuing education, therapeutic certification and is in good standing. Licensee may practice in the state of Oregon.
      "INACTIVE" Status = Licensee is current with regard to appropriate fees. Additional requirements must be met in order for licensee to practice in Oregon.
      Names of licensees whose licenses have lapsed for any reason are not included on this list.

    DISCIPLINARY ACTION ABBREVIATIONS:
      "COE ONLY 00/00/00" = The date of discipline involving the late reporting or failure to complete continuing education in a timely manner.
        "AOD 00/00/00" = The date of disciplinary action that did not involve continuing education.
      DISCIPLINARY REPORTS OF ACTIONS IMPOSED BY THE BOARD FROM 2000 TO PRESENT

        This page provides links to all the disciplinary actions imposed by the Board from the year 2000 to the present, and some actions prior to 2000. Not all the actions prior to 2000 are available on this page. For details about any disciplinary action that does not appear on this page, please send a written request to the Board for a search to be performed and copies of documents to be made. The request must be accompanied by $20.00 per licensee.

      MEDICAL MALPRACTICE REPORTS that have been submitted to the Board.


      Any request for a written verification of a license must be accompanied by a fee of $20.00, in the form of a check payable to the Oregon Board of Optometry. For example:

        ~ A license is lapsed and the name does not appear on this list. If verification of a previous license is required, a request for written verification must be submitted, accompanied by a fee of $20.00.
        ~ A verification of license must be submitted to another state licensing authority, a request for written verification must be submitted, accompanied by a fee of $20.00.

      Pharmaceutical Certification designations are as follows:
        "D" - Certified to Use Diagnostic Pharmaceutical Agents;
        "T" - Certified to Use Topical Therapeutic Pharmaceutical Agents;
        "AT" - Certified to Use Topical and Non-topical Therapeutic Pharmaceutical Agents;
        "ATI" - Certified to Use Topical and Non-topical Therapeutic Pharmaceutical Agents with injections.
